Optional Practical Training Reporting
(for Employers)

The following form is for employers who have recently employed a Washington University who had been authorized for the 17 month OPT extension. Students should not complete this form.

USCIS regulations mandate that as the employer of one of our students, you must inform the Office for International Students and Scholars at Washington University in St. Louis within 48 hours of the termination or departure of F-1 employees. If you have recently employed an Washington University F-1 international student on OPT, please complete the form below.

Please note: an employee is considered to have departed if the student has not reported for work for a period of five consecutive business days without the consent of the employer.
